Consent to Telehealth
Last updated November 7, 2026
This consent is between you and PinkBridge PC or the other professional practice that provides your care through PinkBridge (the “Practice”) and its licensed healthcare providers (“Providers”). You will be asked to agree to it before your first assessment.
What Telehealth Means Here
Telehealth is health care provided remotely using technology. With PinkBridge, a Provider reviews the questionnaire answers and photos you submit through the website or app, usually without a live conversation (“asynchronous” care). The Provider may also message you, ask for more information or photos, or ask you to join a video or phone visit, including where state law requires one. You will be told the name and credentials of the Provider responsible for your care.
How Software Is Used
PinkBridge LLC provides software, which uses artificial intelligence (AI), that organizes your answers and photos and prepares a draft plan for your Provider. The draft is not a diagnosis or a prescription. Your Provider reviews your information, may change or reject the draft, and makes every treatment decision. You can ask your Provider about how the software was used in your care.
Benefits
Telehealth can let you get care without traveling, at a time that suits you, with follow-up through check-ins.
Risks and Limits
A Provider cannot examine you in person. Photos may not show your skin accurately because of lighting, focus or camera quality, and some conditions cannot be assessed from photos.
Your Provider relies on the information you give. Incomplete or inaccurate information may lead to a wrong assessment or inappropriate treatment.
Reviews and replies are not immediate, and technical problems may delay your care.
Despite security measures, electronic information can be intercepted or accessed without permission.
Prescribed medications, including compounded formulas, can cause side effects such as irritation, redness, peeling or allergic reactions. Compounded medications are not FDA-approved.
Your Provider may decide that telehealth is not right for you and recommend in-person care.
Your Responsibilities
Give complete and accurate information, including your medical history, medications, allergies, pregnancy or breastfeeding status, and your current location, and update it when anything changes.
Submit only photos of yourself.
Use any treatment only as directed, complete your check-ins, and tell your Provider promptly about side effects or worsening skin.
Keep a primary care provider or other clinician for your general health care.
Emergencies
PinkBridge is not for emergencies or urgent problems. If you have a severe reaction, such as swelling of the face, lips or throat, trouble breathing, or a severe rash, stop using the product and call 911 or go to the nearest emergency room.
Your Rights
You may refuse or stop telehealth care at any time, without affecting your right to future care, by telling us or closing your account. You can choose in-person care elsewhere instead.
Your health information is protected as described in the Notice of Privacy Practices, and you can get a copy of your records.
Your state may give you additional rights about telehealth.
Your Agreement
By checking the consent box during your assessment, you confirm that you have read this consent, have had the chance to ask questions, understand the benefits, risks and limits of telehealth and of the software described above, and agree to receive care through telehealth.
